Terrible kings, both real and fictional, are often characterized by their profound cruelty, incompetence, or descent into madness, which leads to immense suffering for their subjects. For example:

REAL - Ivan IV of Russia ("Ivan the Terrible," 1530–1584)

While he established Russia's first standing army and parliament, his legacy is overshadowed by his reign of terror through the secret police (oprichnina). He was known for extreme paranoia and brutal executions, which included the killing of his own son and heir.

FICTIONAL - King Joffrey Baratheon (from Game of Thrones)

Joffrey is the epitome of a sadistic, spoiled, and cowardly boy-king who rules through cruelty and impulse. His terrible acts include ordering the execution of Ned Stark against all counsel, having his father's bastards murdered, physically and psychologically torturing his betrothed Sansa Stark, and killing people (including a prostitute, Ros) for his own amusement with a crossbow.
